Northwest Home Health-Winfield serves as an extension of the physicians' care and the hospital services. Health care is provided in the home and includes a diversity of therapeutic and support services to give patients as much independence as possible in the home. Each patient's plan of care is individualized and includes comprehensive assessments, treatment plan, patient and caregiver education, emergency planning, and a home safety evaluation. Intermittent care is provided to homebound patients seven days per week with primary coverage 8:00 a.m. to 4:30 p.m. weekdays Monday through Friday. Weekend admissions and visits are performed to accommodate patient and caregiver needs. On-call staff is available 24 hours per day to answer questions or provide unplanned visits. We are a Medicare Certified Agency.

QUALIFICATIONS
Medicare:
Homebound or cannot leave home without considerable and taxing effort
Under the care of a physician
In need of skilled, intermittent services
Insurance Plans:
Most insurance plans include home health care
Coverage for services may vary
Each patient's coverage is verified
Medicaid:
Provides coverage for skilled nursing and home health aide service
Allows home health aide services to stand alone
Allows up to 104 total visits per calendar year
